Respected Sir/ Madam,

Respected, I am writing this letter to inform you that I met with an accident on
Thursday, 12th may when coming back from office and which led to some serious
injuries and left hand bone got cracked due to which I have to get surgery on my
hand on 13th may in the evening and doctor has suggest me to take rest for 8 weeks
at least .I will not be able to continue my job responsibilities for 8 weeks. I beg to
state that I need a rest for 8 weeks. I have attached my medical along with it.

Therefore, I request you to kindly issue me leave for 8 weeks. I ensure I will be
joining back at the earliest. Thanking you.
