As per your request submitted through Carbon Health Patient Portal on


OCTOBER 9TH, 2022, here is a letter regarding your visit to our VALLEJO URGENT
CARE CLINIC and your lab results.

On OCTOBER 4, 2022, you visited our VALLEJO URGENT CARE CLINIC to get
a pregnancy test completed. You stated that your last menstual cycle began on JULY
28th, 2022. You also listed the following symptoms: late or absence of menstrual cycle,
fatigue, nausea, vomitting. According to the lab results of your UI test, you are pregnant.
Based on the information given to us, you are estimated to be 10 WEEKS pregnant.

Your next scheduled appointment is on OCTOBER 19, 2022, at 10:40 AM in our


SAN RAFAEL CLINIC, MOLLIE MOORE, PA-C. This appointment will confirm your
pregnancy through ultrasound and give you and your doctor your Estimated Date of
Conception (EDC) and your Estimated Date of Delivery (EDD). More information will
be given to you by your doctor and staff on your appointment date.

If you need to change or cancel your appointment, you can do so by logging onto
your Carbon Health Patient Portal account. It is advised that you do not cancel your
appointment without rescheduling as this is an important clinical visit for you and your
baby. Any questions you may have about cancelation and rescheduling can be answered
by a Patient Success Agent through the Carbon Health Patient Portal. Carbon Health
Patient Portal is a tool that gives patients access to lab results, view past doctor visits,
and to update patient contact information.
