Carrier Guide: Disable FMiP Lock

This form is used to request disabling the Find My iPhone lock on a device when a repair is blocked due to the lock and the customer has provided valid proof of purchase. The carrier partner completing the form needs to include the proof of purchase receipt and provide information about the customer's device including serial number, IMEI, product description and original sales details like date and retailer where it was purchased. All fields on the form are required to be submitted to the appropriate Apple email address in order to process the request to disable the Find My iPhone lock.

This form should be completed when a repair is blocked in GSX due to FMiP device lock and where the customer
has provided the valid proof of purchase proving device ownership. The Carrier Partner should complete this form
and attach the proof of purchase receipt of the product’s original retail purchase.

If you answer no to either of these questions, submit the form only after the customer attempts those steps.
