Study Report

The MRI and MR angiography of Ashok Bagal, a 64-year-old male, showed several findings: small fresh infarcts in the right frontal and parietal lobes as well as the right corona radiata; an area of gliosis in the right temporal and parietal lobes; and severe short segment stenosis of the proximal right internal carotid artery with moderate narrowing of the right posterior cerebral artery and a hypoplastic right vertebral artery.

MRI SCAN OF BRAIN , BRAIN & NECK VESSELS


ANGIOGRAPHY

MRI of the brain was performed in T1 and T2 W signal intensities in multiple planes with FLAIR .
MR Angiography of the brain & neck vessels was performed using 3D TOF MR Angio sequence
of circle of Willis in axial plane . Studies reveal :

Small wedge shaped areas of restricted diffusion is seen in the right


frontal, right parietal lobes, right corona radiata, represent fresh infarcts.
Small near CSF signal intensity area is seen in the right temporal, right
parietal lobe, represent gliosis.
Basal ganglia and thalami appear normal. Corpus callosum appears normal.
Pituitary , supra & para sellar regions are normal.
Visualized cerebellum and the brainstem appear normal.
Ventricular system appears normal.
Basal cisterns & cortical sulcal spaces appears normal.
No mass effect / midline shift / herniation.
Bilateral cerebello-pontine angles are normal. The IACs are unremarkable.
The pineal gland & optic chaisma are unremarkable.
The bony skull appears normal.

Brain & neck vessels MR angiogram and row data reveal ------
Severe short segment stenosis of the proximal part of right internal carotid
artery.
Moderate narrowing of the right posterior cerebral artery is seen.
Hypoplastic right vertebral artery is seen.
Both sided common carotid arteries & carotid bifurcations appear normal.
Left sided intra & extra cranial internal carotid arteries are normal.
Both sided anterior, middle and left posterior cerebral arteries are normal.
Both sided external carotid arteries are normal.
Both vertebral arteries are normal. Basilar artery is normal.
No evidence of aneurysm or AVM seen.

CONCLUSION :
Small wedge shaped areas of restricted diffusion in the right frontal, right
parietal lobes, right corona radiata, represent fresh infarcts.
Small near CSF signal intensity area in the right temporal, right parietal
lobe, represent gliosis.
Severe short segment stenosis of the proximal part of right internal carotid
artery.
Moderate narrowing of the right posterior cerebral artery.
Hypoplastic right vertebral artery.
