AFFIDAVIT OF TEMPORARY CLOSURE

I, MARIA CRUZ, of legal age, Filipino, married, and a resident of Bacolod


City, after having been duly sworn to in accordance with law, hereby depose and
state that:

1. I am the bona fide owner or proprietor of a business enterprise with


registered name XING XING INC. and business address at Bacolod City, 6100;

2. I am executing this Affidavit of Temporary Closure in my aforesaid


capacity;

3. Effective last 2018 up to May 2020, we closed the said business due to
mismanagement and the financial crisis brought about by the pandemic, which has
severely affected our business operations;

4. We have only resumed our operations on June 2020. However, the


employees’ contribution for Pag-IBIG have not been updated because the person
in-charge of the same was not able to turn over the documents to the management
properly;

5. With the business struggling to pull through, the interests of both the
employer and the employees were affected;

6. I am now executing this Affidavit, therefore, in order to attest the truth


of the foregoing facts and for whatever lawful purposes this may serve, specifically
for compliance purposes as required by the Home Development Mutual Fund or the
Pag-IBIG.
