PENETRATION INSPECTION CHECKLIST

1. Guardrails and/or toe boards have been inspected to be strong, rigid and in good condition

2. To remove the entire piece of wire steel mesh to install duct/equipment to occupy the entire opening, all
workers must wear body harness and adequately anchor with fall protection such as guardrails, self-rewinding
fall arrestor, etc.
3. Fall protection-full body harness, anchorage point, safety net, lifelines are provided to workers trained in
the correct use of all these equipment.

4. To cut openings in wire steel mesh, hot work checklist is completed

5. Exclusion or drop zone has been established below the penetration

6. Installed duct/pipes/equipment are secured to prevent dislodge and fall through opening

7. Openings (new and existing) must be covered and secured when work finishes within the same day

8. Never leave openings unattended especially during break times

9. Any incompatible works?
