NCP Activity Intolerance

The patient reported feeling weak and lacking energy. Upon assessment, the patient appeared pale with high blood pressure and rapid heart rate. The nurse diagnosed the patient with activity intolerance related to anemia from chronic renal insufficiency. The nurse planned to help the patient increase activity levels and self-care over 4 hours. Nursing interventions included assessing factors contributing to fatigue, promoting independence, and encouraging rest alternating with activity. After 4 hours the patient was able to participate more in activities and self-care while feeling an increased sense of well-being.

DIAGNOSIS Activity intolerance related to anemia due to Chronic Renal Insufficiency

RATIONALE Erythropoietin production is at the kidneys but due chronic renal insufficiency, there is a decrease in Erythropoietin production and profound anemia results. Lack of hemoglobin Decrease oxygen to tissues Weakness and fatigue

PLANNING After 4 hours of rendering nursing care. The client will be able to: -participate in increase level of activity and exercise -report increase in sense of well being -alternates rest and activity -participates in selected self-care activities

INTERVENTION -Asses the factors contributing to activity intolerance: a. fatigue b. anemia c. fluid and electrolyte imbalance d. retention of waste products e. depression -Promote independence in self-care activities, assist if needed -Encourage alternating activity with rest

RATIONALE -Indicates factors contributing to severity of fatigue

EVALUATION After 4 hours of rendering nursing care. The client was able to: -participate in increase level of activity and exercise -report increase in sense of well being -alternates rest and activity -participates in selected selfcare activities

-Promotes improved selfesteem -Promotes activity and exercise within limits and adequate rest Adequate rest is encouraged after dialysis treatments, which is exhausting to most patients

-Encourage patient to rest after dialysis treatments
