3 VENTILATION
• Are all air conditioner inlets and outlets and extractor fans
clear of obstruction?
• Is there a flow of air to remove fumes, smoke, dusts, etc.,
which are generated from work? (welding, painting, etc.)
4 WASHROOM AND TOILETS
• Are floors, walls, etc., clean and in hygienic condition?
• Are enough towels, toilet paper and soap supplied?
5 POLLUTION
• Is there any area where pollution is taking place uncontrolled?
(oil, fuel spillage, etc.,)
6 STACKING AND STORAGE
• Are stacks safe and tidy?
• Are shelves strong enough to hold the loads?
• Are aisles kept clear, providing safe and easy access to all
stored items?
• Are foods and hazardous substances stored separately?
7 OUTSIDE AREA
• Are work areas, areas under pipe racks, scaffolds, and
between equipment kept free of rubbish, building debris,
cables, hoses and other loose items?
8 SCRAP AND REFUSE REMOVAL
• Are enough bins provided?
• Are rubbish bins emptied regularly?
9 COLOR CODING
• Are all electrical and lifting equipment and mobile equipment
correctly color-coded for the current inspection period?
10 MACHINE GUARDING
• Are all hazardous moving parts within normal reach
completely guarded or fenced off, especially nip points?
11 LABELLING
• Are all containers labeled to show what their contents are?
(fluids, including drinking water)
• Are all hazardous materials and MSDS on records?
12 LIFTING GEAR
• Have you noticed any unsafe equipment? (Specify)
• Have you noticed any defect? (Specify)
13 ELECTRICAL INSTALLATIONS
• Have you seen any electrical equipment with obvious
dangers?
• Are all equipment properly grounded?
• Are proper tags and signs posted?
14 HAND TOOLS
• Have you seen hand tools that are unsafe? (mushrooms on
chisels, loose hammer head on handle, homemade or altered
tools, etc.)
15 PERSONAL PROTECTIVE EQUIPMENT
• Have you seen any PPE or other safety equipment that are not
up to standard? (body harness, gloves, face shileds, masks, ear
plugs, safety shoes, etc.)
16 NOTICES AND SIGNS
• Area all notices and signs clean and in good condition?
• Are all the correct signs posted?
17 FIREFIGHTING EQUIPMENT
• Have you seen any equipment which is not up to standard?
• Are there enough fire extinguishers installed/provided?

• Are all cylinders secured with chain and in upright position?
• Are flashback arrestors, reducers, gauges fitted to oxy-
acetylene sets?
• Are all unused cylinders capped?

• Are fire watch, fire extinguishers, fire blankets used?
19 PNEUMATIC TOOLS
• Are Chicago couplings wired together?
• Are disc and grinder speeds compatible?
• Did you notice anything unsafe about the tools, personal
protection, or handling of the tools?
20 PORTABLE ELECTRICAL TOOLS
• Did you notice anything unsafe about the tools, electrical
cords, guarding, or handling of the tools?
• Are electrical tools properly grounded and insulated?
21 TRENCHES / EXCAVATIONS
• Are the sides of the trenches / excavations sloped or stepped
to ensure safety?
• Are enough and safe access ways provided?
• Are the excavations barricaded and dirt piled 1m away from
the edge?
22 STAIRS, LADDERS AND SCAFFOLDS
• Did you notice anything unsafe on the condition of the
equipment? (Specify)
• Are the equipment inspected and appropriately tagged?
• Did you notice anything unsafe in the use of the equipment?

• Are scaffolds provided with safe access, walk boards, toe
boards, hand and mid-rails?
• Are scaffolds inspected and correctly tagged?
