Defects are to be report to and cleared by the Supervisor / Foreman.
IMPORTANT NOTE: If defect is a safety hazard or requires Immediate repair, shut down machine, attach WARNING DEVICES MON TUE WED THU FRI SAT SUN
an Out of Service tag, advice Supervisor and do not use until supervisor sign / dates cleared by & date. Check horn
