ﹾChief Complaint: body weakness and epigastric pain ; abdiominal bloatedness
History present illness:
- 1 months PTA- confined at Tayug Family Hospital , 1 unit BT done
- 2 days PTA noted bipedal edema
- 1 day PTA- noted puffy face
- Patient is known Diabetic for 20 years and dx as a case of CKD until 1 day PTA -patient experienced
generalized body weakness accompanied by epigastric pain, no radiating with feeling of fullness . No
consult
- Few hours PTA-experienced symptoms persisted an patient’s wife decided to brought here at our
institution hence the admission
Past History :
- 2014 -Dx Hypertension -BP- 190/ taking Amlodipine
-2020- noted ↑ creatinine at Generika Pharma -monitored Crea
- Jan.2023 consulted a Nephro in Tayug Family Rx. Amlodipine 10mg OD , Losartan 50mgOD- pt.self
regulated his intake of Amlodipine 10mg BID
Final Diagnosis:
To consider Ileus

Cardiovascular: dynamic precordium , (+) murmur , regular rhythm , apex beat at 6th ICS
Abdomen: globular, soft , non-tender , normoactive bowel sound , slightly distended
Musculoskeletal : (+) pitty grade 1 , bilateral bipedal edema
Neurological: No Neuro Deficit

MEDICAL ABSTRACT
Course in the Ward:
Upon admission patient placed on bed comfortably at ER, assessed vital signs, seen and examined by Attending
Physician signed consent for hospitalization. Laboratory work–ups and other diagnostic procedures requested
and done then relayed to Attending Physician . Then hooked aseptically to an IVF of PNSS 1L .Meds started:
Furosemide IV, Sodium Bicarbonate tab , Amlodipine tab , Epoetein SQ, Omeprazole IV , Ceftriaxone IV ,
Antamin IV , Kremizine sachet , Ketoanalogue tab ,Vitamin B-Complex , MVT + Iron ,tab , Renakult tab,
NaHCO3 IV , Calcium gluconate IV ; Other oral meds: Amlodipine tab , Clonidine tab , Calcium tab.Patient
referred to a Nephrologist for co-management. Patient refused Hemodialysis , Abdominal Ultrasound , ABG.
Patient underwent Blood Transfusion 2 units of Whole Blood . Monitored vital signs accurately. Patient
ensured safety, referred accordingly.
