MEDICAL CERTIFICATE

This is to certify that Mr. King Defante 28 y/o, male; single of Barangay Alima, Quezon
City, came in to this clinic on December 4, 2016 for Medical check-up and urgent
operation to amputate his legs and other severe injuries. He is advised to stay in the
hospital for three weeks to four weeks until I find it safe for him to go home.

On Physical Examination:
Vital Signs: BP 120/80 Temp. 38.5 C RR 21/min HR -80/min.
HEENT: E/N
Chest/Lungs: harsh breath sounds, + crackles at left BLF
Cardiovascular: RRR, Dynamic precordium
Abdomen: soft, NABS, no organomegaly
GUT: E/N
Neuromuscular: E/N
Extremities: E/N
*His legs were needed to be decapitated to prevent other infections.*

Diagnosis: Decapitated Legs

Hence is advised rest for three weeks to four weeks until signs and symptoms disappear.
Medications given, Amoxicillin 500 mg TID, Ambroxol BID, Mefenamic Acid,
Paracetamol 500 mg 1 tab every 6 hrs.prn for fever. This certification is issued for
whatever medical purpose it may serve him best.
