Student Teacher Interview Consent Form

Dear Mrs. Rose Ann Manansala,

I hope this letter finds you well. We are writing to inform you that Meryll Soriano, a student
teacher from Pampanga Colleges, is currently undergoing practical training at Sta. maria
elementary School. As part of their educational experience, they will be conducting interviews
with parents/guardians of students.

The purpose of these interviews is to gain insights into the student's home environment,
educational background, and any additional information that might enhance the student
teacher's understanding of the learner.

Your participation in this interview is entirely voluntary, and you have the right to decline or
withdraw your consent at any time without consequence. The information collected will be kept
confidential and used solely for educational purposes.
