The patient reported difficulty hearing out of their right ear. An assessment found irritability in the ear, altered hearing, and impaired communication such as shouting and being out of focus. The diagnosis was disturbed auditory sensory perception related to presbycusis. The nursing intervention plan was to instruct the patient on safe ear cleaning techniques and non-verbal communication. After 2 hours the patient was able to verbalize understanding of the teaching and demonstrate non-verbal gestures.
