PETITION
UNDERSIGNED PETITIONER, unto this Honorable Court most
respectfully states that:
1. He possesses the eligibility for commissioning as Notary Public
as shown by the following personal circumstances:
a) Nationality : Filipino

2. He is a member of the Philippine Bar under Roll of Attorneys
No. 82792, having been duly admitted thereto on 31 May 2022.
Attached hereto is an original of the Certificate of Membership in the
Philippine Bar as ANNEX “A”;
3. He has paid his IBP annual dues for the year 2022 as shown by
an original of the IBP Official Receipt No. 229965 hereto attached as
ANNEX “B”;
4. He has likewise paid the required Professional Tax for the year
2022 in Tacloban City, Leyte, as shown by an original of the Official
Receipt No. 8271310 dated 9 June 2022 which is hereto attached as
ANNEX “C”;
5. He holds office at Justice Romualdez Street corner Sto. Niñ o
Street, Tacloban City, Leyte. Photographs of the petitioner’s office are
hereto attached as ANNEX “D” and ANNEX “D-1”;
6. He is a resident and a natural born citizen of the Republic of the
Philippines since birth;
7. He has not been convicted, much less charged, with any offense
involving moral turpitude or any criminal or civil offenses for that
matter. Attached hereto as ANNEX “E” is the NBI Clearance issued by
the National Bureau of Investigation;
8. He is a member of good standing of the Philippine Bar as shown
by the following certifications:
a. Certification of No Pending Administrative Case issued by
the Office of the Bar Confidant of the Supreme Court is
hereto attached as ANNEX “F”,
b. Joint Certification as a Member of Good Standing and of
Good Moral Character issued by IBP Leyte Chapter is hereto
attached as ANNEX “G”;
9. He now desires to be appointed and commissioned as Notary
Public within the territorial jurisdiction of the Regional Trial Courts of
Tacloban City, Leyte, for the term ending on 31 December 2023;
10. Three (3) of Petitioner’s passport-size colored photographs
with name and signature at the bottom thereof are attached in this
petition as ANNEX “H” and a separate sheet of paper forms part of this
petition containing three (3) specimens of petitioner’s signature is
likewise attached as ANNEX “I”;

11. He has not applied for nor was he issued a Commission as a
Notary Public in any other jurisdiction for the current year;
12. He has paid in full the proper filing fees;
13. He undertakes to submit or report to the Office of the Clerk of
Court copies of all documents and deeds notarized by him;
14. He possesses all the qualifications and none of the
disqualifications and desires to be appointed as a Notary Public for
Tacloban City, Leyte, within the territorial jurisdiction of this Honorable
Court.
PRAYER
WHEREFORE, Petitioner most respectfully prays that he be
appointed and commissioned as Notary Public for and in the City of
Tacloban, Leyte, within the territorial jurisdiction of this Honorable
Court. It is likewise prayed that Petitioner be authorized to purchase his
notarial seal for the purpose.

my personal knowledge or based on authentic records;
4. That I hereby certify under oath in this petition as follows:
a. I have not theretofore commenced any action or filed any
action or claim involving the same issue in any court, tribunal
or quasi-judicial agency and, to the best of my knowledge, no
such other pending action or claim is pending therein;
b. If there is such other pending action or claim, a complete
statement of the present status thereof; and
c. If I should thereafter learn that the same or similar petition has
been filed or is pending, I shall report that fact within five (5)
days therefrom to the court wherein the aforesaid petition or
initiatory pleading has been filed.
