attached as Annex “A”;
4. She has never been charged nor convicted of any crime
involving moral turpitude. She is further attaching the
Certification of no pending administrative case against her
from the Office of the Bar Confidant of the Supreme Court
as Annex “B”; Certificate of Good Moral Character issued
by two (2) executive officers of the local chapter as Annex
“C” of this Petition; Clearance from the Office of the Clerk of
Court as Annex “D” and Clearance from the National
Bureau of Investigation as Annex “E”;
5. She is not a government employee nor is she employed in
any government owned and controlled corporation, and she
is not commissioned as Notary Public in any corporation;
6. She desires to be commissioned as Notary Public for and in
the City of _____ and possesses all the qualifications and
none of the disqualifications for the said office;
7. In compliance to the requirements of the Honorable Court
for Notarial Commission, attached hereto are the following
annexes and made an integral part of this petition:
a. Three (3) Specimen Signatures as Annex “F”;
b. Three (3) pieces of Passport sized ID picture as Annex
“G”;
c. Copy of IBP Membership Payment Receipt as Annex
“H”;
d. Copy of the PTR Receipt as Annex “I”
PRAYER
WHEREFORE, foregoing premises considered, it is respectfully
prayed of this Honorable Court that after compliance with the
prescribed requirements and hearing, the notarial commission of
your petitioner, for and in the City of Baguio, be given.
