Petitioner, Juril B. Patio and unto this Honorable Court,
respectfully avers : that
1. Petitioners personal qualifications are narrated hereunder, viz :

2. He maintains office at G/F GMC Plaza Bldg, Legazpi Ext., corner
M.J. Cuenco St., Cebu City, Philippines.
3. He has been admitted to the practice of law on April 27, 2015, as
shown in the Certificate of Membership in the Philippine Bar
which contains his Roll of Attorneys No. 63966. A copy of the
Certificate of Membership in the Philippine Bar issued by the
Office of the Bar Confidant is hereto attached and marked as
Annex A.
4. He is a member of the Philippine Bar in good standing with a
clearance from the office of the Bar Confidant. Attached and
marked as Annex B is the copy of the clearance issued by the
Office of the Bar Confidant dated May 13,2015.
5. He is likewise a bona fide member of the Integrated Bar of the
Philippines with IBP No. 0997508 and that no administrative or
disbarment case was ever filed against him. A copy of the
certification of the Integrated Bar of the Philippines showing his
good moral standing is hereto attached and marked as Annex
C.
6. He has paid the requisite membership dues for CY 2015 with the
official receipt. Attached and marked as Annex D is the copy of
the official receipt for the payment of the membership dues of the
Integrated Bar of the Philippines.
7. He has not been convicted or charged with any offense involving
moral turpitude. Attached and marked as Annex E is the copy of
the clearance issued by the Office of the Clerk of Court of the
Regional Trial Court of Cebu.
8. He is of good moral character. Attached and marked as Annex
F is the copy of the certification issued by the Integrated Bar of
the Philippines Cebu City Chapter certifying to the effect that
petitioner is a person of good moral standing.
9. He has been issued clearance from the National Bureau of
Investigation which shows that petitioner has no derogatory record.
Attached and marked as Annex G is the clearance issued by the
National Bureau of Investigation.
10.He shall faithfully abide by the Rules on Notarial Practice (A.M.
No. 02-8-13 SC).

PRAYER
WHEREFORE, it is respectfully prayed before this Honorable
Court that upon consideration of the foregoing facts and circumstances, a
Notarial Commission and a Certificate of Authorization to Purchase a
Notarial Seal be issued in favor of the Petitioner.
