AFFIDAVIT

I, ANKIT CHAUBEY S/O Mr. Parshuram Chaubey residence of H. No.-D-194, New


Ashok Nagar, Delhi-110096, do hereby Solemnly affirm and declare as under:-

1. That I am the present resident of above said address.

2. That my Roll No. 1900950100014.

3. That the original Fee Receipt of my ward in course B. Tech CSE from
Mahatma Gandhi Mission College of Engineering and Technology, sector
-62, Noida, in the year 2019 have been burned/misplaced/lost anywhere.

4. That the said deposit receipt has been misplaced by me and if I get the said
receipt in future I shall return the said receipt to the College authority.

5. That I undertake to indemnify the College authority due to any loss or


damage incurred in the future due to that lost receipt.

6. That the above said contents are true.
