Dear Mrs. Jackson

Re: Mrs. Marjorie Mellors, aged 77

I am writing to refer Mrs. Mellors, a 77-year-old widowed lady, who was admitted to our ward after
being involved in an accident and she has suffered multiple leg injuries and closed colles’ fracture of
right radius, and she needs your further care.

Mrs. Mellors is a friendly and active person who is living alone and she is helped by her son and
neighbors as she often forgets to take her medication. She has a background of osteoporosis and
hysterectomy and she has no history of allergies.

During hospitalization, Mrs. Mellors underwent an open reduction internal fixation surgery, her wrist
wound stitches monitored with cold compress and her leg was treated with daily dressing changes. A
series of medical procedures were done and all were found in normal range (pulse, temperature,
respiratory rate, blood pressure, blood analysis and wrist X-ray). She was prescribed naproxen 500mg,
orally, twice a day, and an intravenous antibiotic.

Her post-operative stitches will be removed on 20th May 2018 at Orthopaedic Outpatients and three
weeks of physiotherapy outpatients from 15th May 2018 was programmed, to achieve full motion of
wrist, fingers and forearm . Following this, she will require assistance with daily activities (showering,
dressing) and keep the wrist dressings clean and dry. Mrs. Mellors was advised to moisturizing her
wound area on legs with Medihoney ointment, for two days and pain medicine if she needs.
