AFFIDAVIT OF GOOD CONDUCT AND NON CULTISM

I, AGBEKO ELISHA KEHINDE, Male, Christian, Yoruba and Nigerian Citizen residing at No. 6,
Aloba layout, honeymoon off Ondo road, Akure, Ondo State, Nigeria. I do hereby solemnly,
sincerely and conscientiously make oath and declare as follows:

1. That I am the declarant herein.

2. That I am 100 Level Student (Fresher) with Jamb Registration Number: 202331278166HA,

Adekunle Ajasin University, Akungba-Akoko, Ondo State of Department of Political

Science, Faculty of Social Science.

3. That I have not belonged to any Cultism or evil gangs in life and I will not belong to any one

throughout my education period.

4. That I will abide with school authority law and orders and be of Good conducts throughout

my education period.

5. That I implore all authorities concerned to take note of the above facts and act accordingly

6. That this affidavit is needed for record and official purposes.

7. That I swear to this affidavit in good faith, believing same to be true, correct and in

accordance with the Statutory Declaration Laws of Ondo State, Nigeria, 2006.
