➢ According to Patient- Patient says “ merko koi pareshani nhi hai na he koi problem hai. Yeah
log merko faltu ider le aye.. merko gussa aa rha hai yeah jab dekho mere he peche pade rhthe hai
bina vajhe he. Yeah log mai pagal hu sabit krna chathe hai.meri chutti kaar dho ider se.”

➢ According to Informant- Patient’s father says “ phechle 6 mahinne se yeah bilkul sotha nhi
hai aur sab pr bhout gussa krtha hai aur gali be detha hai. Appni safai ka dyan be nhi raktha , patha
nhi kasi kasi ajjeb bathe kartha hai itna badi badi bathe krtha hai, but zada karch krne laga hai sath
yeah hamesha ghumtha yeah thaltha he rhtha hai ek jaga arram se nhi beita hai. Yeah ghar k
saman fenk detha hai boltha hai yeah gandi aur karab hai. Patha nhi kyun yeah hum logo pr bhout

➢ HISTORY OF PRESENT ILLNESS:

Mr. Manu, 38 years old Married male came with his father and wife through emergency bases and
got admitted in RML hospital on 01/05/23 at 3:15 pm under section 89 involuntary admission.
Onset of illness insidious, duration of illness is 15 years, course of illness episodic, intensity is
increasing.
Precipitating factor- Non-Compliance of medication and family history of psychiatric illness.

and also share his thoughts related to it. He is excited to tell things. The conversation ended well.

INTROSPECTION

I was able to maintain rapport with the patient, understand and interpret the information that patient
gave me during the interview. Gain confidence in conducting the interview. Identified the
problems patient is facing. Communication skills get improved. Learned to use different
therapeutic techniques with client.

Patient greeted the nurse, looks confident and relaxed. He maintained eye to eye contact with
nurse, he become comfortable and clarify and explain his points in detail. He also listens attentively
when something was explained but was also showing interest in expressing his feelings and
actively listen to the information and suggestions.

I was able to maintain rapport with patient, understand and interpret the information that patient
gave me during the interviews. Gain confidence in conducting interview. Learned to use different

➢ OVERALL SUMMARY
Mr. Manu, 38-year Male, married, graduate, was active during interview. He shared his problems
and his concern regarding his treatment during the course of interview. We have talked on different
topics, we have started with the basic introduction phase and established a trustful rapport to each
other than on day 2nd we talked about general discussion to identify the main problems, on 3rd day
we discussed about drug compliance, on day 4th we talked about anger and stress management and
at last day we discussed about maintaining personal hygiene. Patient was equally participating in
discussion, he also actively listens to the information provided. He accepts his mistakes and gave
assurance that he will work on them and improve his behaviour. The interview was terminated on
mutual understanding.
