1. On the above-stated date and time, a vehicular accident occurred along Roxas
Boulevard, Manila, involving two vehicles a white Toyota sedan with plate number
ABC 123 and a blue Honda SUV with plate number XYZ 456. The accident occurred
near the intersection of Roxas Boulevard and United Nations Avenue.
2. According to eyewitnesses, the Toyota sedan was traveling at a high speed when
it attempted to overtake another vehicle. In the process, the sedan collided with the
Honda SUV, causing both vehicles to spin out of control and come to a stop on the
roadside.
3. Casualties and Injuries:
The driver of the Toyota sedan, identified as Mr. Juan Dela Cruz, sustained
minor injuries and was treated at the scene by responding medical personnel.
The driver of the Honda SUV, identified as Ms. Maria Santos, also sustained
minor injuries and received medical attention at the scene.
4. Response and Actions Taken:
Upon receiving the report of the accident, personnel from the nearest police
station, as well as medical emergency services, were dispatched to the
scene.
Police officers arrived promptly and secured the area to ensure the safety of
motorists and pedestrians.
Traffic flow along Roxas Boulevard was temporarily disrupted to facilitate the
clearing of the accident site and the investigation process.
5. Further Investigation:
The investigating officer gathered statements from witnesses and collected
evidence at the scene to determine the cause of the accident.
Both drivers were subjected to breathalyzer tests to ascertain their sobriety at
the time of the incident.
The vehicles involved were towed to the nearest police impound for further
examination and investigation.
6. The preliminary investigation suggests that the accident was caused by over
speeding and reckless driving on the part of the driver of the Toyota sedan. Further
investigation is underway to determine any additional factors contributing to the
incident.

Narrative:
Prior to the accident, a collision occurred on Roxas Boulevard involving a
white Toyota sedan and a blue Honda SUV. The Toyota, driven by Mr. James Bond,
was speeding and attempted to overtake another vehicle, leading to a head-on
collision with the Honda SUV driven by Mr. Manuel Roxas. Both drivers sustained
minor injuries and gave their statements at the scene.
Both drivers gave their statement and both were advised to submit estimate of
damage on both vehicles.
This report is for the purpose of insurance claim only.
