Malagnat National High School will be joining an outside of the school activity which is
the Provincial Nutrition Month Contest hosted by Provincial Health Unit (PHU) to be held on
August 01, 2023 at Kalinga Astrodome, Bulanao, Tabuk City, Kalinga.
This parental consent is being issued to you to inform and to request permission for you
to allow your son/daughter regarding the conduct of the intended activity. Signing this form,
means you are giving your consent so as to justify that the Administration or the
coordinator/adviser/teachers shall not be liable for any untoward incident/s arises from the
student’s negligence which is beyond the control of the Administration or personnel in charge
supervising them.

Considering the purpose that will derived from the participation in such activity for the
interest of our son/daughter, we voluntarily waive any claim against DepEd employees and
personnel in-charge, for any untoward incident which may occur. God forbids beyond control in
the course of his/her participation after all precautionary measures and exhaustive effort has been
taken by the person in-charge.
