1. I am the DAUGTHER of WILFREDO MAGLINTE REBOJO who was baptized in

2. Due to unknown reason, the baptismal records of WILFREDO MAGLINTE REBOJO

MAY 15, 1963 at the Roman Catholic Church of HOLY CHILD PARISH, BATO, LEYTE by REV. FR.
FRANCISCO FEGI and was inadvertently not entered in the Baptismal Register of the parish;
That we know all the foregoing facts because we are friends of the family;
That we are not related to the family either by consanguinity of by affinity and that we are not
interested in any way or manner for which this affidavit is being executed.

fraud or committing any crime and that we hold ourselves liable for any consequence that may arise from
