AFFIDAVIT
I, Lorena G. Lat, of legal age, single, Filipino citizen, and a resident of Plaridel, Lipa City,
Batangas, after having been duly sworn in accordance with law, do hereby depose and
state that:
1. I am the daughter of LODINA M. GUERRERO as shown in my Certificate of Live
Birth issued by the Philippine Statistics Authority;
2. In my Baptismal Certificate is my mother’s name as entered in “LUDINA
GUERRERO”;
3. In the Certificate of Confirmation, issued by Our Lady of Parish Lourdes in Iyam,
Lucena City, Philippines my mother’s name appears as “LUDING GUERRERO”;
4. The discrepancy in the Certificate of Confirmation and Baptismal Certificate was
due to a typographical error;
5. I am executing this affidavit to attest to the truth of this fact and to request the
correction of the first name in the Certificate of Confirmation and Baptismal
Certificate.
