AFFIDAVIT OF LOSS
I, LESLIE T. ABAD, of legal age, Filipino, single, and currently residing at
Barangay Mandaragat, Puerto Princesa, Palawan, after having been sworn in
accordance with law, hereby deposes and states the following:
1. I am the owner and holder of Driver’s License identification card issued on
May 12, 2023 by the Land Transportation Office, Puerto Princesa City,
Palawan;
2. I used to put the abovementioned identification card in my wallet, inside
my bag;
3. Sometime in July 2024, while doing my house chores, I noticed that my bag
was open, I looked for my Driver’s License ID inside my bag but I can no
longer locate it;
4. Despite diligent efforts exerted to locate or recover my Driver’s License ID,
all of my efforts were to no avail and as of this moment, I have not located
the same, such that I fear that those have been lost beyond recovery;
5. I now consider my abovementioned Driver’s License ID as irretrievably lost
and can no longer be found despite diligent efforts to locate the same;
6. I execute this affidavit freely and voluntarily, to attest to the truth of the
foregoing.

AFFIDAVIT OF DISCREPANCY
I, LESLIE TABANG ABAD, of legal age, Filipino, single and a resident of
Barangay Mandaragat, Puerto Princesa City, Palawan, under oath depose and
say that:
1. My full name as appearing on my Birth Certificate is LESLIE TABANG
ABAD. It was erroneously written in my SSS identification card, my first name
as “LESLEY” instead of “LESLIE”;
2. The fact that my SSS identification card is entered erroneously which
is different to my original name in the birth certificate;
3. I am executing this Affidavit to say that my correct first name is
“LESLIE” and not “LESLEY” as appearing in my record on the
abovementioned file;
4. I am executing this Affidavit to attest to the truth and veracity of the
foregoing statements.
