I, RUBEN R. PADILLA, of legal age and with residence at Blk


1, Lot 1, Brgy. Sta Monica, Puerto Princesa City, having been duly
sworn, depose and say:

That I, RUBEN R. PADILLA, married and of legal age,


currently working as an analyst at the Palawan Diagnostic
Laboratory, Puerto Princesa City, Palawan.

That I personally know that the person named herein in


the complaint and the person whom I personally know are one
and the same person.

That he is my classmate in High school, and a friend of


mine.
