AFFIDAVIT OF DEMOLITION
I, JEANELYN DUNGO-SEVILLA, of legal age, married, Filipino and
resident of Gatbuca, Calumpit, Bulacan, after having been duly sworn to in
accordance with law, do hereby depose and say:
That my deceased Father Higino Dungo is the declared owner of a
building covered by Tax Declaration No. 2006-02500 located at Gatbuca,
Calumpit, Bulacan;
That on or about year 2012, we have caused the demotion of the said
building for the reason that we were no longer in need of our pottery
business;
That there is no more any improvements covered by the said Tax
Declaration and we are requesting for the cancellation of the aforesaid Tax
Declaration by the proper government office;
That I am executing this affidavit to attest to the veracity of the facts
contained herein and for all legal intents and purposes it may serve.
IN WITNESS WHEREOF, I have hereunto set my hand this 14 th day of

been duly sworn to in accordance with law, do hereby depose and say:
1. That I am the registered owner of a building covered by Tax
Declaration No. 2018-18019-09892 located at Tabang,
Plaridel, Bulacan;
2. That on or about year 20__, we have caused the demotion of
the said building for the reason that we were no longer in need
of our former house;
3. That there is no more any improvements covered by the said
Tax Declaration and we are requesting for the cancellation of
the aforesaid Tax Declaration by the proper government office;
4. That I am executing this affidavit to attest to the veracity of
the facts contained herein and for all legal intents and
purposes it may serve.
