PARENTAL CONSENT

I, the undersigned, as the parent/legal guardian


of______________________________, Grade _______ student of
_________________________, ______________, Cebu, do hereby give my full
consent and approval for my child to participate in the Numeracy and
Literacy remediation, starting this Monday, December 09,2024 at 4:00-5:00
in the afternoon.
I have considered the benefits that my son or daughter will derive
from his/her participation in this program provided that due care and
precaution will be observed to ensure the comfort and safety of my
son/daughter and that DepED employees and personnel may not be held
responsible for any untoward incident that may happen beyond their control.
I acknowledge that I am signing this freely and voluntarily and
intend this by my signature to be a complete and unconditional release of all
liability to the greatest extent allowed by law.
