Guidelines For Patient Registration Based on ABHA Health Id.
This document lists Health ID functionalities added in e-Hospital. Users are advised to
use this document as a reference to register the patient through “National Health ID”.
Summary of New Features:

5. Patient want to Create ABHA.
6. Patient don’t want to Create ABHA.
1. Shared Profile
Option1=> Self Registration:
Page link: Patient Registration->NDHM Counter QR Code.
Step1: Click on “NDHM Counter QR Code” Link.
Step2: Select Counter from the dropdown.
Step 3: Click on ‘Generate QR Code’ or Click on “Print icon” to generate QR code
for the respective counter.
Step 4: After clicking on the ‘GENERATE QR CODE ‘Button the self-registration
slip will be generated along with the QR code.
Step5: Scan above QR code through NDHM PHR mobile appends hare the date to
precede self-registration.
Step6: Go to New Registration link and click on “Shared Profile” icon.
Step 7: Select respective counter from dropdown and click on “Search”
button.
Step8: Click on select button. The patient demographic data will be displayed in
the OPD template.
Step9: After click on save button Registration slip will be generated in the front of
screen.
2. Search Health ID
Option2=> Enter Health ID and Search:
IfthepatientdoesnotprovidethehealthcardbutremembershishealthIDor health ID
number, and then selects this option.
Page link: Patient Registration -> New
Registration.Step1: Click on “Search Health ID”icon.
Step2: Enter Health ID and click on “SEARCH” button.
Step 3: Click on “MOBILEOTP or Aadhar OTP”
Step 4: Click on Verify OTP.
NOTE: - For new ABDM registration is not asking for OTP in revisit. While in old
registration in NDHM it’s asking for OTP.
Step5: Once the OTP verified Patient demographic data will be fetched and displayed
in the respective fields.
Step6: After click on “save” button, OPD slip will be generated.
3. Scan Health ID
Option3=> Scan with QR Code:
If the patient comes with his Health Card, scan the QR code from the health card
using a QR code scanner. The scanned details will automatically appear on the page.
Page Link: Patient Registration->New Registration.
Step 1: copy scan data marked as per given image.
Step2: After scanned QR Code Click on “NEXT” button and press ok.
Step3:- It will display demographic details of the user.
Step4:-: After fetching data select department, Doctor Room and then save the details and
then the registration slip will be generated.
4. Revisit with Health ID
Option4: Revisit existing Patients through Health ID:
Page Link: Registration->Search
Step1: In the search page, search patients by different category.
Step2: From the list select a patient for revisit and click on select button and then click Re-
Visit link.
Step3: After fetching data select department, Doctor Room and then save the details and
then the registration slip will be generated.
5. Patient wants to Create ABHA
Step 1 :- User can create his ABHA ID from this link with his mobile linked with
Aadhaar.
If user has already ABHA id then it will move to registration page.
Step2:- After filling the mandatory details clicking on ‘Next’ button for OTP
verification.
Step3:- firstly verify the Aadhar otp.
Step4:- Then provide the mobile otp for verification.
Step5:- It will display Aadhar demographic details of the patient.
Guardian name to be filled manually at the time of registration.
Step 6:-After fetching data select department, Doctor Room and then save the details and
then the registration slip will be generated.
6 Patient don’t want to Create ABHA
Step 1:-If Patient doesn’t want to Create ABHA then it will move to new registration page
Step 2:-Now fill the data manually and registered the patient.
