AFFIDAVIT OF LOSS
I, JOHN MAYNARD M. UDAG, of legal age, married and a resident of Phase 3, Block 2,
Lot 2, Family Village Resources, Brgy. Poblacion 5, Gen. Mariano Alvarez, Cavite, after having
been duly sworn in accordance with law, hereby depose and say:
1. That I possessed and owned a Student-Driver’s Permit No. D31-22-S39897 issued by
LTO-Robinson’s Pala-Pala, issued on 2022/04/26 valid until 2023/04/26, which I put
inside my bag wherever I go;
2. That on March 6, 2024, when I was about to process my renewal of my Student-Driver’s
Permit, I immediately looked for my Student-Driver’s Permit inside my bag but to my
surprise it was no longer there, upon knowing it was missing I immediately exerted effo
rts to find it but my efforts were futile as it was nowhere to be found, I must have been
mistakenly thrown it as trash;
3. That the said above mentioned Student-Driver’s Permit is now lost and beyond
recovery;
4. That the said Student-Driver’s Permit has not been confiscated by the Land Transportat
ion Office (LTO) , Police or other Traffic Enforcers for any Traffic Violation;
5. That I executed this Affidavit to attest to the fact of loss with my further request to Lan
d Transportation Office to accept this Affidavit for the purpose of issuing a new Student-
Driver’s Permit.
IN WITNESS WHEREOF, I have hereunto affixed my signature this 7th day of March, 2024,
at Gen. Mariano Alvarez, Cavite.
