Worksite Observation Report

Date of Observation: 24/10/2024

Worksite Activity: Excavation, Cable Pulling, Working at Height, Machinery Use

1. Cable Pulling Work

Observation: Cables were not secured properly during pulling, leaving some loose on the ground, which
could lead to tripping or damage. Workers were not wearing gloves, increasing the risk of cuts and
abrasions.

Action Taken:

- Directed workers to secure cables properly during the pulling process and keep them off the ground.
- Reminded the team to wear gloves to prevent injuries.
- Asked the supervisor to conduct a safety briefing on proper cable handling techniques.

2. Working at Height

Observation: Workers were performing tasks at height without proper fall protection, such as harnesses
or guardrails. The scaffolding in use was not checked for stability before work began.

Action Taken:

- Stopped the work immediately and instructed workers to use fall protection equipment, including
harnesses and lanyards.
- Requested a safety inspection of the scaffolding to ensure stability and security.

3. Machinery Use

Observation:

Some machinery, including excavators and forklifts, lacked safety plugs on electrical outlets, posing
electrical hazards. Additionally, some machines were missing safety guards during operation.

Action Taken:

- Shut down machines lacking safety plugs and ensured proper plugs were installed.
- Instructed operators to verify that safety guards are in place before using machinery.
- Conducted a quick inspection of all machinery to confirm they are safe and in good condition.
4. PPE Compliance

Observation:

Some workers were not wearing required PPE, including safety helmets, gloves, and high-visibility vests,
especially near active machinery.

Action Taken:

- Reminded all workers about the importance of PPE and conducted a quick PPE safety session.
- Ensured everyone received the necessary equipment.
- Instructed supervisors to monitor PPE compliance regularly.

5. Excavation Work

Observation:

Excavation work is ongoing, with workers digging in trench areas. Debris is scattered around, posing a
tripping hazard. There is also a lack of proper signage to warn workers and visitors about the excavation
site.

Action Taken:

- Instructed workers to clear debris regularly to avoid tripping risks.


- Placed warning signs around the excavation area to alert workers and visitors of the danger.
- Recommended securing the excavation site with barriers to prevent falls or injuries.

Conclusion
Immediate actions were taken to address the safety concerns on-site, including improving
housekeeping, enforcing PPE compliance, ensuring machinery safety, and securing work at height.
Continued monitoring and follow-up actions will help maintain these safety measures.

2. Ensure regular equipment inspections.
