Induction Form
What is an Induction Form?
An Induction Form is used to record that a new employee or contractor has received
induction into the company’s safety systems and procedures. It should be completed as the
inductee is going through the induction process, and checks off each element of the
induction process once training and understanding of each element has been achieved.
It is wise to customise the induction process to be as specific to the company’s needs. Some
general items to consider having in the form are:
Company OHS policy and procedures,
Roles and responsibilities within the company structure,
Hazard identification and control,
Hazardous material register and MSDS
Accident ,Incident and Hazard forms
The use of Personal Protective Equipment
Safe Work Method Statements
Toolbox Talks
The induction process is one that needs to be constantly reviewed to ensure that each new
employee is totally familiar with the company’s policies, procedures, rules and other
company specific training needs.
