Stock card question

MANAGEMENT OF DRUGS AND OTHER HEALTH FACILITY EQUIPMENT

Introduction: Record keeping in health care set up, is fundamental not only for management purposes
but also accountability and follow up purposes.

One of the most effective ways for record keeping is using a stock card, which stipulates the health
details of the facility, health unit code, item description, pack size, item code no, any special storage
conditions, date of issue, average monthly consumption, maximum stock levels as well as minimum
stock levels. The stock card helps to identify quantity of supplies in/out of stock, losses, balance at hand
as well as their expiry date.

Activity five: a) - filling a stock card

The following information was obtained with from Kyamuhunga Health Centre III, code 002 located in
Igara west HSD, Bushenyi District.

The health centre received 5 tins of paracetamol each containing 1000 tablets on 12/1/2019 (with an
expiry date of Dec 2019) and batch number C 43/09.Paracetamol is stored in a cool dry place according
to the pharmacists’recommendations.The unit had consumed over 12,000 tablets over 12 months. The
maximum stock level allowed by their medicine management system is 5 month stock while the
maximum is 2 month stock.

By the time they received the aforementioned quantity, they had a stock of 5000 tablets.

On 12/2/2019 the stores issued paracetamol tablets as follows (voucher No. 001/2019) a) 200 to
maternity

b) 500 to OPD

c) 300 to inpatient ward

On 12/3/2019 issues were as follows (Vou.002 / 2019)

a) 500 to maternity

b) 200 to in patient ward

On 12/4/2019 the stores in-charge discovered that 100 tablets had been destroyed by water and the
DHO requested for 500 tablets which were given out to Kyabugimbi Health centre IV. On the same day
issues (Vou. No. 003/2019) were as follows:

a) OPD 200

b) Inpatient 300

On 15/5/2019 another consignment of 2 tins of paracetamol with an expiry date of Aug. 2022 with a
batch number of C48/09.

On 12/6/2019 the centre received two 1000 tin tablets of paracetamol for a community outbreak from a
nearby facility (Swazi H.C.II)
