Procedure:

Fitting for an upper partial acrylic denture involving the following teeth:

 Left side: Lateral incisor (22) to second molar (27)


 Right side: Lateral incisor (12) to second molar (17)

This procedure was performed in an NHS practice. Before beginning, I confirmed the patient’s
medical history, ensuring it was up to date. The patient had no known allergies, including latex
allergies.

Patient Details:

Age: 78-year-old female

Mobility: Uses a walking stick, has a hearing impairment, attended independently.

Medical History: No known allergies or ongoing medications.

Lifestyle: Non-smoker, does not consume alcohol

Preparation of the Clinical Area:


I began by wearing the appropriate uniform and washing my hands in compliance with HTM01-
05 guidelines before starting the procedure. Next, I switched on and checked the functionality of
all necessary equipment, including:
• Computer system
• Dental chair
• Dental light
• X-ray units
• Compressor
• Autoclaves
• Aspiration equipment
To ensure infection control compliance, I disinfected the following surfaces using viricidal
wipes:
• Dental chair
• Light handles
• Worktops
• Keyboard
• X-ray units
• Spittoon
Additionally, I flushed the dental water line for 2 minutes using aspirator cleaners, as required by
infection control protocols.

Instruments and Materials Preparation:


For the denture impression procedure, I prepared the following:
 Instruments: Aspirator suction tube, dental mirror, probe, tweezer and flat plastic.
 Straight Handpieces and trimming burs.
 Materials: 3-in-1 tips, articulating paper and gauze.
 Additional Items: patient mirror and self-aspirating syringe prepared in case the clinician
required it.

All instruments and materials were arranged systematically on a sterile tray for easy access
during the procedure.

PPE Prepared:
• For Clinician: Gloves, mask and dental face visor.
• For Myself: Gloves, mask and protective glasses.
• For Patient: Bib and goggles.

Assisting During the Treatment:


I ensured all instruments and materials were systematically organized for the clinician’s easy
access.

Instrument Handling & Support:


 Passed instruments efficiently as required.
 Adjusted the dental light for optimal visibility.
 Set up the model, denture, articulating paper, straight handpiece, and trimming burs as
needed.
Patient Monitoring & Support:
 Helped the patient get comfortable before treatment.
 Adjusted the dental chair to accommodate mobility limitations.
 Regularly checked on the patient’s well-being, offering reassurance throughout.
 Explained each step clearly and addressed any questions or concerns.
 Encouraged steady breathing to keep the patient relaxed.
Post-Treatment Assistance:
 Assisted the patient in safely sitting up and ensured they felt stable before standing.
 Provided a mirror for the patient to assess the fit and appearance of the denture.
 Checked for any areas of discomfort or pressure and relayed feedback to the clinician.
 Offered guidance on inserting and removing the denture properly.
 Provided verbal and written aftercare instructions, including cleaning and maintenance
advice.
 Encouraged the patient to practice speaking and swallowing with the new denture.
Reassured the patient and addressed any concerns before they left the clinic.

Follow-up Appointment:
Before the patient left, I scheduled a follow-up appointment to assess comfort, function, and any
necessary adjustments to the denture.

Aim of the Procedure:

The aim of the denture fitting procedure is to ensure the patient receives a well-fitting, comfortable, and
functional prosthesis that restores their ability to chew, speak, and smile with confidence.

Reflective Account
Strengths:
1. Instrument Handling & Clinical Assistance: I efficiently passed instruments, adjusted the
dental light, and prepared the required materials, ensuring a smooth workflow.
2. Post-Treatment Support: I helped the patient understand denture care and encouraged her
to practice speaking and swallowing with the new denture.

Weaknesses:
1. Assessing Denture Fit & Occlusion: Observed the clinician using articulating paper to
check the bite but felt uncertain about identifying pressure points and areas needing
adjustment.
2. Communicating with a Hearing-Impaired Patient: While I made an effort to speak clearly
and slowly, I could improve my communication techniques by incorporating visual aids
and written instructions more effectively.

Action Plan:
1. Observe more denture fittings and ask the clinician to explain the adjustments in detail.
2. Improve Communication with Hearing-Impaired Patients, try to use written aftercare
instructions with clear diagrams to reinforce verbal explanations.
3. Ensure I always face the patient while speaking to make it easier for them to understand.
4. Practice explaining denture care instructions clearly and concisely.
