Colostomy Care Procedure

1. Preparation

- Gather all necessary equipment:


- - Gloves (sterile and non-sterile)
- - Warm water and soft gauze/cotton balls
- - Skin barrier/protectant wipes
- - Adhesive remover wipes
- - Colostomy pouch (drainable or closed-end)
- - Stoma measuring guide
- - Scissors for cutting the wafer
- - Disposable bag for waste
- Explain the procedure to the patient and ensure privacy.
- Wash hands thoroughly and wear non-sterile gloves.

2. Removing the Old Colostomy Pouch

- Carefully peel off the old colostomy bag, starting from the top while supporting the skin.
- Use adhesive remover wipes if necessary to prevent skin irritation.
- Dispose of the used pouch in a disposable bag.

3. Cleaning the Stoma and Surrounding Skin

- Inspect the stoma for color, swelling, or signs of infection. A healthy stoma is moist and pink/red.
- Clean the stoma and surrounding skin using warm water and gauze or soft cloth.
- Avoid using soaps with alcohol, fragrance, or oils, as they can cause irritation.
- Pat the area dry with a clean gauze pad.

4. Preparing and Applying the New Colostomy Bag

- Measure the stoma size using a stoma measuring guide.


- Cut the wafer to fit the stoma properly, ensuring a snug fit without causing pressure.
- Apply a skin barrier wipe around the stoma to protect the skin.
- Place the new colostomy pouch over the stoma and press gently to secure it in place.
- Ensure the pouch is properly sealed to prevent leakage.

5. Post-Procedure Care

- Dispose of used materials properly in a designated waste bag.


- Remove gloves and wash hands thoroughly.
- Educate the patient on proper colostomy care, dietary considerations, and signs of
complications.

6. Patient Education and Follow-Up

- Encourage the patient to empty a drainable pouch when it is one-third full.


- Teach the patient about foods that may cause gas, odor, or blockage.
- Advise the patient to check the stoma and surrounding skin daily for any changes.
- Recommend follow-up with a healthcare provider if any complications arise, such as skin
irritation, excessive leakage, or signs of infection.
