Colostomy care
Scenario: Mr. Harish with the history of colon cancer, in his 3rd post-operative day after
colostomy, the stoma was oozing. Assess the stoma and perform the colostomy care in steps.

1. Identify the patient and explain the procedure to
the patient and how he has to cooperate.
2. Assembles the necessary article: -
A tray containing
a) Mackintosh
b) Long sheet/towel
c) Clean gloves’ cotton swabs and gauze
pieces
d) Wash cloth
e) Water in basin
f) Soap in dish
g) Scissor
h) Skin barrier-Zinc Oxide
i) Bedpan with cover
j) New pouching system
3. Provides privacy and assist patient to a
comfortable Fowler’s or lying position.
4. Washes hands and d o n gloves to prevent
infection.
5. Assesses the location of stoma and the type of
colostomy performed.
6. Place the mackintosh or towel under the patient.
7. Empty the partially filled appliance into the bedpan if
it is drainable pouch
8. Remove the ostomy appliance slowly from the top
by stretching the skintight.
9. Use tissue paper or gauge piece to remove the
excess stool from the stoma.
10. Assesses the stoma appearance and surrounding
skin condition for moist reddish-pink color, blood
supply and secretions.
11. Clean the skin around the stoma with cleansing
agent/ soap according to 1hospital policy but do not
scrub the skin.
12. Gently pats dry for 1-2 min the peristomal skin and
stoma. Apply the skin barrier (Zinc Oxide).
13. Apply the colostomy bag by remove the backing to
expose the sticky side.
14. Applies skin barrier and pouch appliance
together.
a. Select size of stoma opening by using the
measurement guide
b. Trace the same size circle on the back at the
center of the skin barrier.
c. Use scissors to cut an opening ¼ or 1/8inch or
2.5 cm larger than stoma.
d. Remove gauze pad covering stoma. Place
barrier and pouch over the stoma and gently press
out skin smoothing out crease or wrinkles. Hold
the pouch in place for 5 minutes.
15. Closes the pouch if it is drainable by folding the end
upward and using clamp or clip according to
manufacturer’s direction.
17. Makes the patient comfortable & document the
time of the procedure, condition of stoma,
peristomal skin and patient reactions to procedure.
18. Dispose off the used pouch, swabs in yellow bag,
clean supplies, discard gloves and wash hands.
