Subject- Request for Attendance Exception Due to Mental Health Condition

Respected Sir
I am writing to formally acknowledge my low attendance and take full responsibility for it. I
understand and respect the college’s attendance regulations, and my parents are fully aware of
my situation. Despite my best efforts, I was unable to attend college throughout January due
to severe relapse of anxiety neurosis, which led to extreme depression.

As per my doctor’s evaluation and prescription, my condition required urgent medical

attention, making attendance impossible. While I understand that medical reasons are not
usually accepted for attendance shortages, I urge the administration to recognize that mental
health is as crucial as physical health. Denying me the opportunity to appear for my exams
despite my medical documentation would be deeply unjust.

I have always been a dedicated student and have consistently performed well academically.
My mental health struggles should not deprive me the chance to prove my capabilities. I have
attached my doctor’s note, prescription details, and an undertaking signed by my
parents and me confirming my situation.

I sincerely request you to consider my case with due seriousness and grant me permission to
appear for my examinations. Please let me know if any further documentation or discussion is
required.

Looking forward to your response.

Best Regards
Shreeta Shah
