AFFIDAVIT OF MARRIAGE

We, both the bridegroom and bride the bridegroom viz, Mr.

Sachin Rambadan Jaiswal, and Mrs. Khushbu Sachin Jaiswal

both residing atL-168/3, Kariya Yadav Chawl, Gandhi Nagar,

Near Railway Fatak, Kalwa-400605 do hereby swear solemnly

on oath of our religion that:


1. Identity of Affiant: We, Mr. Sachin Rambadan Jaiswal,

age 32, bearing aadhar card no. 882173501317 and Mrs.

Khushbu Sachin Jaiswal, age 26 bearing aadhar card no.

679713615492 and we are citizen of India. We are

competent to testify to the facts stated herein.

2. Marriage Information: I Mrs. Khushbu Sachin Jaiswal

born on 14.05.1998 married to Mr. Sachin Rambadan

Jaiswal, born on 05.05.1993. We were married on

24.04.2023 in Gram Ravuam Post, Mohammadabad,

Sipah Thana, Doharighat, Mau, Uttar Pradesh.

3. Marriage Ceremony: We re-iterate and confirm and

published at the time of our wedding pursuant to this, our

wedding was solemnized in presence of our near and

dear relatives, friends and guests from both the sides in a

very simple way, professing as per HINDU customs, rites

and ceremonies and we have printed invitation card. We

confirm that our wedding is solemnized with our natural

consent, understanding, knowledge, faith belief and

without any compulsion, force, inducement, allurement,

coercion or influence on us

4. Purpose of Affidavit: This affidavit is being submitted

and made for the purpose of the validity of the marriage

for legal purposes, marriage license application,


municipal corporation, teshsil office, sub-registrar of

marriage, or any other government department etc.

5. Affiant’s Statement: I hereby declare under penalty of

perjury that the information provided above is true and

accurate to the best of my knowledge and belief.
