I,______________________________________,agree to

participate in a research study entitled “FROM AN INERT SOUL

TO A PASSIONATE HEART: NARRATIVES OF ENGLISH TEACHERS IN

TEACHING LISTENING” conducted by Jacob, John Michael, and

Farrah, 4th year students taking Bachelor of Secondary

education major in English at Iloilo State University of

Fisheries Science and Technology, under the direction of

Glysa Marie T. Regidor, Thesis adviser. I understand that my

participation is voluntary. I can stop taking part of

without giving any reason, and without penalty. I can ask to

have all the information about me returned to me, removed

from the research records, or destroyed.

The purpose of this study is to determine the role of the

listening skills, the challenges, and strategies employed by

the English teachers in the classroom.

If I volunteer to take part in this study, I will be asked

to do the following things:

1. Agree to become a member of the research team that

Mr/Ms.Jacob, John Michael, and Farrah will form.

2. Serve as a participant.

3. Participate in research that will narrate my teaching

experiences.

4. Participate in the research process such as interview and

research questions, data gathering, and data analysis.

5. Answer questions related to the research as to my

- teaching experiences.

I also give consent to audio record on one-on-one interview.

During the data collection and analysis, I also give access

to the researchers to collect all the relevant information

narrated. I allow the researchers to keep all the records of

the data collected from this research, which will be

destroyed 6 months after the study.

I expect no risk involved in this research. However, I

understand that my participation in this research will eat

much of my time. Nevertheless, I see that the benefits I

gain in learning a hands-on qualitative research far

outweigh my discomfort as a participant. Other than the

learning experience to work closely with Mr/Ms. Jacob, John

Michael, and Farrah in this research, I expect no

compensation as a result of my participation in the research

process.

I also give consent to Mr/Ms. Jacob, John Michael, and

Farrah to make results of this study public through journal

publications, paper presentations, and other forms of public

dissemination of research results. I understand that results

of this study will be made public with my identity

anonymous. Unless otherwise required by law, I also

understand that the results of my participation will be

strictly confidential outside our research team and will not

be released in an individually identifiable form.
