AFFIDAVIT OF KINSHIP

I, MARILOU ALVAREZ LORZANO, of legal age, Filipino citizen, married and a


resident of Bgy. Poblacion, Aborlan, Palawan, Philippines, after having been duly sworn to
in accordance with law, hereby depose and say:

1. That I am one of the siblings of EMILY S. ALVAREZ who died on May 12, 2020,
at Aborlan, Palawan, Philippines;

2. She died without family; I am her nearest surviving kin and relative;

3. That I am executing this affidavit for the purpose of requesting a copy her Death
Certificate and all civil registry documents in that regard, with the Philippine
Statistic Authority and Municipal Registrar Office;

4. That I am executing this Affidavit not for any remuneration whatsoever but to
attest to the truth and veracity of the foregoing statements for whatever legal
purpose it may serve him.
