AFFIDAVIT OF KINSHIP

I, ISIDORA M. POTESTADES, of legal age, married, Filipino citizen,


with a postal address # La Virgen Street, Brgy., Longos, Kalayaan, Laguna after
having been duly sworn to in accordance with law, depose and voluntary state:

1. That I am next of kin od deceased BIENVINIDA R. MACAWILI, who


died last March 28, 2025 at Kalayaan, Laguna, being her sister;

2. The late BIENVINIDA R. MACAWILI (my sister) was still single at


the time of her death;

3. Our parents JUSTINO D. MACAWILI and CASTORA R.


MACAWILI were also both deceased;

4. As a sister, I am one of the nearest surviving kin and relative;

5. I am executing this Affidavit for the purpose of requesting Death


Certificate of my late sister (Bienvinida) and other documents related therto at
LCRO-Kalayaan, Laguna and PSA to be used in filling burial and insurance claims
at Social Security System;

6. Finally, I am executing this affidavit to attest to the truthfulness of the for


going declaration and for legal intents and purpose this may serve.

AFFIANT SAYETH NONE.
