I, K.V. RAMESH, S/o. Dr. K.S. Seetharam Sharma (late),
Hindu aged about 62 years, office at No.26, Law Chambers, High
Court buildings, Chennai - 600 104, do hereby solemnly affirm
and sincerely submits as follows:-

{ 1 am the petitioner 's counsel herein and as such [ am well
acquainted with the jacts of the case.
2. 1 respectfully submit that my client’s wife P.Hemavathi
who has worked as Staff ‘Nurse in Prima:ry Health Center
at deferent places and she was died on 25.02.2000.
3. I further submit that my client has made representation
for death cum retirement benefits of his v.vife P.Hemavathi
to 2nd respondents on 25.03.2022. Due to my client’s ill
health he unable to present the letter to 2nd respondent
herein in time.
4.1 further submit that the above matter was returned by
the registry in Sr. NO.121665 / 2023 on 13.10.2023 for
pompliance of the quarries. I says that the bundle was
mingled along with other contempt appli;cation and the
bl.;ndle was traced out on 13.12.2023 and complied the
above said quarries and defective represen:ted before this
Hon’ble Court.

for the genuine reason stated as above for defective in
representation. No prejudice will be caused against the
respondent otherwise the petitioner will be pernil.
