AFFIDAVIT OF LOSS

I, AMALIAH SEPI SULTANBATAO, of legal age, married, a Filipino citizen, and presently
residing at 1021 7TH ST., DIMALUNA II, MSU-MAIN, MARAWI CITY, LANAO DEL SUR ,
after having been duly sworn in accordance with law, hereby depose and state that:

1. I am the lawful owner and holder of a valid University ID and Library ID issued by
Mindanao State University- Main and the University Library respectively ;
2. On or about March 5, 2025, I discovered that my said ID was missing/lost and
despite diligent efforts to locate it, I have been unable to recover the same;
3. The said ID may have been lost when I was travelling bound to Manila at the
Laguindingan International Airport, I apparently dropped my wallet.
4. I am executing this Affidavit to attest to the truth of the foregoing facts and for the
purpose of reporting the loss and requesting for the issuance of a replacement ID.

IN WITNESS WHEREOF, I have hereunto set my hand this 14th day of April , 2025 at
Marawi City , Philippines.
