This is to certify that Mr. Ram Shrestha. a permanent resident of Dagi, Bhimeshwor 9,
Dolakha, Bagmati Province, Nepal has submitted an application to this office for the
verification of relationship with his family members.

Whereas, after following the necessary official enquiry and conducting public enquiry from this
Municipality, it is found that the following person has following relationship. Therefore, it is
