This is to certify that Mr. Kushal Mishra, permanent resident of Sunbarshi Municipality,
Ward No. 01, Morang District, Koshi Province, Nepal has submitted an application to this office for
verification of relationship with his family members.
Whereas, after following the necessary inquiry and conducting public inquiry from this office, it
is found that the following person have following relationship which are mentioned below. This
relationship certificate is issued after verifying the relationship in accordance with the local government
operation act, 2074 B.S. (2017 A.D.), Chapter 3, Section-12, Sub-section 2, Clause(E-1).
