PSYCHOLOGICAL ASSESSMENT REPORT

NAME: Mohammad Razeen DATE: 15/4/2024

AGE: 13 Years 4 months
SEX: Male
REASON FOR REFERRAL: IQ assessment

BRIEF HISTORY:
Razeen is a 13 year old boy who came for an IQ evaluation. Razeen was going to Parampara School in
Bangalore. Currently he has not been attending school for the last few months due to medical reasons. His
parents report that he has difficulties in academics and his academic performance has been poor. There is a
history of hypoxic ischemic encephalopathy, seizures and birth asphyxia. Developmental delay was
reported.

His ability to understand concepts is poor and he has difficulty with academics. He forgets what he has
studied easily. He has difficulty writing and makes spelling mistakes. He has repeated classes to help him
cope. In his previous school he had difficulties sitting in one place but now his sitting tolerance has
improved. He has also studied in multiple schools. No other behavioural problems were reported.

TEST ADMINISTERED:
➢ Wechsler Intelligence Scale for Children (WISC IV – India)
➢ Nimhan’s Battery for Specific Learning Disability (Level 2)

BEHAVIOURAL OBSERVATIONS:
Razeen was friendly during the assessment. His eye contact was good. He was doing the tasks given to him.
He could understand instructions given though some instructions had to be repeated or simplified.

TEST FINDINGS:

On the WISC IV Razeen obtained a Full Scale IQ of 61 indicating that he falls in the borderline level of IQ
(Average IQ range is between 90 and 109). The Full Scale IQ (FSIQ) is derived from a combination of ten

subtest scores and is considered the most representative estimate of global intellectual functioning.
However, there is scatter in his scores indicating a discrepancy in abilities between domains of ability.

The WISC IV also gives scores on 4 indices. The Verbal Comprehension Index is designed to measure
verbal reasoning and concept formation. The Perceptual Reasoning Index is designed to measure nonverbal
concept formation, visual perception and organization, simultaneous processing, visual-motor coordination,
learning, and the ability to separate figure and ground in visual stimuli. Working memory index measures
the ability to sustain attention, concentrate, and exert mental control. Processing speed is an indication of the
rapidity with which a child can mentally process simple or routine information without making errors.

IMPRESSION AND RECOMMENDATIONS:

Razeen’s parents were counseled regarding the findings of the assessment. His current level of functioning
in various areas was discussed in detail. His poor academic performance and learning difficulties can be

attributed to his intelligence level. The need to tailor expectations according to his abilities was stressed. He
will benefit from schooling with a flexible syllabus and intense input.

Additionally, the following suggestions can be followed to help Razeen:


• Academically the focus can be on improving his basic reading, comprehension, and mathematical
abilities.
• Training regarding essential life skills and improving self- help skills will be beneficial to Razeen.
The focus should be on making him more independent and self- sufficient.
• Use of visual and other study aids will help his learn and reinforce learned material
• Liberal use of positive reinforcement during learning will motivate Razeen to persist in the tasks and
try to learn it.
• Defining goals and sub goals clearly will be helpful.
• Revision of skills already learnt at regular intervals is recommended
• Vocational training can be considered at a later stage.
• Constructive criticism should be used, and the child’s abilities need to be kept in mind when
evaluating his performance in various aspects.
