1.Original & Photocopy of Grade 12 SHS Card
2.Certificate of Good Moral Character
3.PSA/NSO Birth Certificate (Original)
PSA/NSO Marriage Certificate (for married female students only)
4.Medical Certificate is temporarily waived. This will be
required upon resumption of face to face classes

1. Original & Photocopy of Transcript of Records
2. Certificate of Transfer Credential
3. Items 2 to 4 in the above list
Procedue After Interview
1.Log-in to your Applicant Access Module through your ID Applicant Number for the status of your Interview.
2.A NOTICE OF ADMISSION is attached if qualified or passed. Kindly read carefully the Notice of Admission.
All the necessary procedure is given for your guidance.
3.Kindly fill out the Pre-Registration Form attached for slot reservation. Have your Application ID Number
reflected in your email subject for easier tracking. Send your filled out registration form to
admissionprereg@tup.edu.ph within five (5) working days after scheduled interview.
Failure to send back this Pre-Registration Form to admissionprereg@tup.edu.ph will mean forfeiture of your slot.
You will no longer be accommodated in the course and you will again start the process should you be interested
to apply again.
This is also not a guarantee for your enrollment as you still need to submit original documents within the
period given.
4.We will be communicating as well to your school as to your acceptance in the University and initially requesting
for your Documents.
5.Prepare for the documents needed to complete your enrollment. Incomplete documents will not be processed
6.Documents for Submission. You are given a particular period to comply and submit the original documents to
complete your enrollment. Failure to do so would again mean forfeiture of your slot.
a.Form 138-A (Grade 12 for SHS graduate)/Honorable Dismissal and Transcript of Records (for
transferee).

d.Submission of medical certificate is temporarily waived. This will be required upon resumption of face
to face classes.
The submission of Original Documents shall depend on the Quarantine Status:
Should our Quarantine Status permit:
Original copies of the documents (as listed in No. 1) shall be submitted personally to the University Registrar.
Please take note of the period you are supposed to submit your original documents as reflected in your NOTICE
OF ADMISSION.
If with Strict Quarantine Status:
*Original copies of the documents (as listed in No. 1) shall be submitted to the University Registrar through
courier or personally bringing the listed documents to the University and dropped in the designated boxes at
GATE 2 The listed documents must be enclosed and sealed in a long brown envelop and write the following
information.

8.A Certificate of Registration (COR) shall be issued to confirm your enrolment. The COR contains the enrolled
subjects with corresponding schedule.
