I, Special Agent Sean McLain, with the Louisiana Bureau of Investigation, certify under oath based on the
information outlined below, that there is probable cause to believe Candace M. Taylor on or about the
date(s) of 03-01-2021 to 11-27-2024 did commit:
1 Count(s) of 14:79.9-- Government Benefits Fraud-- (Felony)

within this State and Parish at: 628 N. 4th Street Baton Rouge, Louisiana 70802 in East Baton Rouge
Parish and the jurisdiction of the 19th Judicial District Court, contrary to the form of the statutes of the
State of Louisiana in such case made and provided, and against the peace and dignity of the same, in that
the following did occur:
Between 2020 and 2024, Candace Taylor engaged in a series of actions to apply for and receive Medicaid
benefits through misrepresentation, concealment, and non-disclosure of required information.
On May 29, 2019, she applied the alias "Candace Sailor," in which she reported her income as $1,900
biweekly from Options and claimed no dependents. This application was denied due to exceeding the
income threshold. Less than one year later, on March 23, 2020, she reapplied using the same misspelled
name, alleging that she had no income and listing a child, Jaydon Thomas, as a dependent. Notably,
Jaydon was already receiving Supplemental Security Income (SSI) and Medicaid and was not her
biological child; nevertheless, her application was approved.

A post-enrollment review conducted in September 2020 revealed unreported income in the Louisiana
Workforce Commission system, yet her coverage remained active under Public Health Emergency (PHE)
protections. Throughout the subsequent two years, Taylor failed to respond to multiple Requests for
Information (RFis) pertaining to proof of income, identity, and deductions. Despite this lack of response,
her coverage persisted due to PHE policies and a reported pregnancy in 2022.
