DETAILED ENQUIRY REPORT BY COMMITTEE

ENQUIRY REPORT
A) Introduction-
A fatal electrical accident was reported by Sub Divisional Engineer, Udharbond Electrical Sub Division,
APDCL. The address and date of occurrence of the accident was Kashipur Part II, Near Kashipur CRPF Camp
and 03rd July 2025 respectively. In pursuance to the Office Order No 15 Dated 04 th July 2025 an enquiry
committee is constituted by the Chief Executive Officer, Cachar Electrical Circle. The members of the
committee are as follows
1) Sri Arijit Das, DGM, (R&P), Projects o/o the Chief Executive Officer, Cachar Electrical Circle,
APDCL
2) Md Salim Uddin Ahmed, AGM, Cachar IRCA Division, APDCL
3) Dibyendu Das, AGM, Silchar T&C Division, APDCL

B) Statement of the Sub Divisional Engineer, Junior manager, Line man, Representative of M/S JBCO
Powertech Pvt Ltd and Proprietor of M/S AARVEE Electricals involved in the work-
1) Statement of Md Saharab Mohsin Barbhuiya, Sub Divisional Engineer, Udharbond Electrical Sub
Division, APDCL
The Exhibit I enclosed in the enquiry report is the statement of the Sub Divisional Engineer, Udharbond
Electrical Sub Division. In the statement he mentioned that a company named M/S JBCO Powertech Pvt
Limited was executing the work of utility shifting near the location of accident on 3 rd July 2025. The
fatal accident involving Late Saddam Hussain, aged approximately 23years s/o Mukharul Hoque a
resident of village- Rangeswari, District – Kamrup Rural was due to the gross violation of safety
protocol by the supervisor and the labour of the company involved in overhead line shifting as they tried
to deenergized LT side of a distribution transformer on their own without any intimation to or
permission from APDCL officers and staffs (during the lunch break). The labour removed three number
of fuses of a circuit but didn’t disconnect the fuse of another circuit in which he was supposed to work.

2) Statement of Sri Kunal Banerjee, Junior Manager, Udharbond Electrical Sub Division, APDCL
The Exhibit II enclosed in the enquiry report is the statement of the Junior Manager. In the statement he
repeated the same statement as that of the Sub Divisional Engineer.

3) Statement of Sri Mewa Lal Rabidas, LM II, Udharbond Electrical Sub Division, APDCL
The Exhibit III enclosed in the enquiry report is the statement of the Line Staff. In the statement he
informed that Sri Kunal Banerjee, Junior Manager, instructed him to deenergize the overhead 11kV and
LT line to shift the overhead line and a 25kVA DTr at Kashipur Part II. The work commenced at
11:30AM approximately after disconnecting the jumper of 11kV Kashipur Feeder. He also informed to
Sri Gyan Parida, Deputy Manager of JBCO Power Tech Pvt Limited and Md Amir Uddin Laskar, Site
Supervisor that the nearby 100kVA DTR is charged. He left the site and during his absence the worker
disconnected the fuses of a circuit of 100kVA DTR without informing him and came in contact with
charged line as the 2nd circuit of 100kVA DTR remained energized. He also stated that Sri Gyan Priya
Parida informed him over telephone that a labour came in contact with charged LT line and accordingly
he immediately shut down the 11kV Kashipur feeder. He immediately informed the matter to SDE and
JM of Udharbond Electrical Sub Division.

4) Statement of Md Amir Uddin Laskar, Site Supervisor of JBCO Powertech Pvt Ltd
In the Exhibit IV he stated that he contacted the Lineman and the line was deenergized for the utility
shifting work. He was sent to bring Disc and Pin for the overhead line shifting work by Sri Gyan Priya
Parida, Deputy Manager of JBCO Powertech Pvt Limited. While leaving the site he informed that there
are two lines and one is energized, he also suggested that the work is to be carried out in presence of
Lineman of APDCL. But Sri Gyan Priya Parida decided to continue work by disconnecting the fuse of
charged DTR in absence of APDCL`s representative. He received a call from Mr Gyan, Deputy
Manager while he was in Lathigram that a worker got electrocuted while working.

5) Statement of Sri Gyan Priya Parida, Deputy Manager, JBCO Powertech Pvt Ltd
In the Exhibit V he stated that shifting of the Overhead Line is being executed by their company in
coordination with Udharbond Electrical Sub Division, APDCL. On 3 rd July 2025 as per schedule the
work was started in coordination with Sub Division. He informed that APDCL`s empaneled contractor
was not available on that day and hence the work was carried out by workers hired by M/S JBCO
Powertech Pvt Limited due to urgency of the work. He informed that he was not present in site but
reached at the spot within 2 to 3 minutes after receiving the information of the incident. He also
informed that there is no fault from the side of APDCL and accordingly the Company has compensated
the family for the loss amounting Rs 5,40,000.00 (Rupees Five Lakh Forty Thousand Only). He had
enclosed the money transfer message screenshot and the Affidavit of amicable settlement stating that the
case is closed.

6) Statement of Sri Shomyojit Biswas, Proprietor of M/S AAR VEE Electricals


In the exhibit VI he informed that the work of utility shifting was awarded to the contractor vide
Permission Number: APDCL/CAR/CEO/CEC/T-59/25-26/155 Dated 6 th May 2025 from the office of
the Chief Executive Officer, Cachar Electrical Circle, APDCL and vide work order number -
JBCO/HO/2025/726 Dated 18th April 2025 from M/S JBCO Powertech Pvt Limited.
He also informed that the contractor executed work by engaging Sri Sonojit Biswas, Electrical
Supervisor and Sri Dibyendu Saha, Site Supervisor in coordination with the Udharbond Electrical Sub
Division, APDCL. But on 3rd July 2025 both the supervisor was out of station and the utility shifting
work was on halt for few days.

The M/S JBCO Powertech Pvt Limited had engaged his own labour and the shutdown was accorded to
the company without informing the empaneled contractor. The deceased labour was not engaged by
empaneled contractor and the work was not executed by him. He was ignorant in this matter.
DETAILED ENQUIRY REPORT BY COMMITTEE
C) Chronological Sequence of the incident on 3 rd July 2025.
The committee enquired all the staffs, officers, empaneled contractor and representative of M/S JBCO
Powertech Pvt Limited. The official documents were also verified and the chronological sequence of the
incident is as follows
1) It is found that the work namely “Utility Shifting of 33kV, 11kV & LT line including DTR under the
scheme of Package -1 : Four Lanning of Silchar -Dhaneri section from existing chainage km 263+800 of
NH 37 to km 12+920 of NH -306 (Design CH 0+000km to Ch 20 +000km) on Silchar – Vairengte-
Sairang Road in the state of Assam under Bharat Mala Pariyojna, under Udharbond ESD , Silchar ESD
II and Sonai ESD” was authorized by M/S YFC -TTC (JV) to APDCL empaneled contractor M/S AAR
VEE Electricals , Itkhola Road, Silchar. The permission of the same is accorded vide Permission No
APDCL/CAR/CEO/CEC/T-59/2025-26/155 Dated 6 th May 2025 from the Office of the Chief Executive
Officer, Cachar Electrical Circle, APDCL. The permission was granted along with 6 numbers of
compliances to be followed during execution of the work.
2) The work of utility shifting was carried out by M/S AAR VEE Electricals in coordination with the EPC
contractor and Udharbond Electrical Sub Division, APDCL at various locations as per estimate on
various date.
3) Incident on 3rd July 2025- The empaneled contractor informed that his electrical supervisor and site
supervisor were out of station on that particular day and hence informed the M/S JBCO Powertech
Private Limited that the work will temporarily on halt, but due to urgency of the work the company
engaged their own hired workers to continue the overhead line shifting at Khasipur Part II without
informing the empaneled contractor.
4) It is observed that a 25kVA DTR was shifted by the gang of worker under supervision of Line man II,
Junior Manager of APDCL and Site Supervisor, Deputy Manager of M/S JBCO Powertech Pvt Ltd. The
jumper of the 11kV Khasipur Feeder was disconnected at 11:00 Hours (approximately) near a 100kVA
DTR in a manner that the 100kVA DTR remained charged and the DTRs after the jumper was
deenergized.
The work of 25kVA DTR shifting along with overhead LT and 11kV Lines were completed successfully
and there was a lunch break. The line staff namely Sri Mewa Lal Rabidas, Line Man II also proceeded
for the lunch break while leaving the site he informed the site supervisor that the LT lines emanating
from the nearby 100kVA DTR is charged and the work must be resumed in his presence.
However, the employees of M/S JCBO Powertech Pvt Limited continued work in the absence of the
representative of APDCL by disconnecting the fuses of 100kVA DTR. The mentioned 100kVA DTR is
having two numbers of 3phase 4 wire, 440V circuits. A circuit is towards the location of accident and
another circuit is on the other side. The circuit on the other side having three numbers of fuse on each R,
Y and B phase. The circuit towards accident site is having a common fuse for R phase for both circuit
and separate fuses for Y and B phases of the two circuit. The enclosed diagram will provide the details
of the site.
The site supervisor namely Md Amir Uddin Laskar and Sri Gyan Priya Parida, Deputy Manager made a
contradicting statement, however either the site supervisor or the Deputy Manager of the company
instructed the worker to disconnect the fuse of the 100kVA DTR and three numbers of fuse were
disconnected by the worker but the two numbers of fuse towards the site of accident remained closed
and two phase remained charged. The worker namely Late Saddam Hussain son of Mukharul Hoque,
Age- 23 climbed the pole and came in contact with the live LT lines resulting into a fatal electrocution.
The time of accident was 16:00 Hours approximately.
DETAILED ENQUIRY REPORT BY COMMITTEE
The Deputy Manager and site supervisor of the company immediately informed the matter to the
Lineman, JM and SDE of APDCL and the 11kV Feeder was deenergized as soon as possible.
5) The team of Cachar Police arrived at the site and controlled the law-and-order situation and sent the
dead body of the worker for postmortem and at around 20:30 Hours the power supply was restored after
necessary rectification and both the 25kVA and 100kVA DTRs were charged.
6) The contractor compensated the family of the deceased worker by paying the compensation amount of
Rs 5,40,000.00 (Rupees Five Lakh Forty Thousand Only) and also submitted a copy of affidavit stating
amicable settlement between the family member and M/S JBCO Powertech Pvt Limited.

D) Conclusion –
It is hereby concluded from the statement of followings
1) Line staffs, JM and SDE of Udharbond Electrical Sub Division, APDCL
2) Site supervisor and Deputy Manager of M/S JBCO Powertech Pvt Ltd
3) Proprietor of M/S AAR VEE Electricals, Empaneled APDCL`s contractor
And also, after site visit by the members of the committee on 15 th July 2025 it is gathered that the overhead
11kV and LT line along with 25kVA DTR was being shifted on 3 rd July 2025 near CRPF Camp at Kashipur
Part II as per approved estimates and necessary permission by APDCL.
However, the following lapse were observed in the work
1) The shifting work on 3rd July 2025 was executed in absence of the APDCL`s empaneled contractor
2) There was no safety gear, safety helmet, gloves and harness belt provided to the worker.
3) It is also confirmed that there was an unauthorized de energization of circuits of 100kVA DTR in
absence of representative of APDCL and this was the main cause of fatal electrocution.

The M/S JBCO Powertech Pvt Limited appointed by YFC-TTC (JV) to execute the utility shifting had
compensated the family of the deceased worker as per condition number 4 of the permission no
APDCL/CAR/CEO/CEC/T-59/2025-26/155 Dated 6 th May 2025.
Sri Gyan Priya Parida, Deputy Manager of M/S JBCO Powertech Pvt Limited had accepted there was no
fault of APDCL and its representative.
In the enquiry it is affirmative that the there is no fault from the perspective of APDCL. However, it is to be
ensured by the Sub Divisions during the execution of work that the terms and conditions mentioned in the
permission from the office of the Chief Executive Officer, Cachar Electrical Circle, APDCL must be complied
during the execution of the work in near future and safety gears must be provided by the contractor to the
worker at the site.
