AFFIDAVIT OF LOSS

I, JANNIS T. APOSTOL, of legal age, single, Filipino and a resident of


Tuguegarao City, Cagayan after having been duly sworn to in accordance
with law depose and state that:

1. I am an account holder of Land Bank of Philippines with Savings


Account No. 5726038246 and BDO Savings Account No.
013090028705;

2. Just this morning, my wallet was lost where I used to place my ATM
Card;

3. It is unfortunate that I did not recover the said lost wallet and
earnest efforts have been exerted to locate the same but all proved
futile, thus, it is presumed to have been lost for all intents and
purposes;

4. I have executed this Affidavit to attest to the truthfulness of the


foregoing statements and to facilitate the issuance of another
Landbank and BDO ATM Card.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 7th day of


January 2025 in Tuguegarao City, Cagayan, Philippines.

6. Just this morning, my wallet was lost where I used to place my ATM
Card;

7. It is unfortunate that I did not recover the said lost wallet and

purposes;

8. I have executed this Affidavit to attest to the truthfulness of the

ATM Card.
